Mondays, Tuesdays and Fridays (except June 15 health center is closed)

• Several Walk-In slots available in the morning! The first 6 children will be seen without appointments.
• Priority will be given to students entering Kindergarten, 6^th & 9^thgrades, students who are new to their school district and students who need sports physicals.
• Child must be accompanied by their parent or legal guardian in order to be seen.

What to bring:

Your child’s recent immunization record

Medical/All Kids Card

Lake County Health Department/North Shore Health Center must be selected as the Primary Care Provider through Illinois Health Connect (or)

• Income for both parents (last current check-stub each for a discount) (or)

• Letter from Employer

*Physical Exam Fees must be paid in full prior to receiving completed physical exam form.
